EFMP Enrollment Form 2792

I have received the EFMP Enrollment Package, I have the Appointment on the 8th of August in Grafenwöhr Germany. I tried to call the health clinic but no luck. My only question is; the package front page said that I need the DD Form 2792. I have never been enrolled to the Army - do I really need this form and how and where exactly can i get it?

Hast Du denn eine Behinderung oder eine chronische Krankheit die Dich dafuer qualifiziert am EFMP Program tielzunehmen oder habt ihr ein Kind was sich qualifiziert? Hier sind die Kriterien dafuer:

EFMP Enrollment Criteria
It is DoD policy that family members who meet the following criteria will be enrolled in the EFMP:

• Potentially life threatening conditions and/or chronic medical/physical conditions requiring intensive follow-up support (such as high risk newborns; patients with a diagnosis of cancer within the last 5 years; sickle cell disease; insulin dependent diabetes) or sub-specialty care.
• Chronic mental health diagnosis (such as bi-polar, conduct, major affective, thought or personality disorders), inpatient mental health service within the last 5 years, mental health services required at the present time, including patients under the care of their primary care manager (PCM).
• A diagnosis of asthma or other respiratory related diagnosis with wheezing which meets one of the following criteria:
  • Routine use of inhaled anti-inflammatory agents and/or bronchodilators.
  • History of emergency room use or clinic visits for acute asthma exacerbations.
  • History of one or more hospitalizations within the past 5 years.
  • History of one or more intensive care unit admissions.
• A diagnosis of attention deficit disorder/attention deficit hyperactivity disorder (ADD/ADHD) that meet one of the following criteria:
  • A co-morbid psychological diagnosis.
  • Requires multiple medications, psycho-pharmaceuticals (other than stimulants) or does not respond to normal doses of medication.
  • Requires management and treatment by a mental health provider(s) (e.g. Psychiatrist, Psychologist, Social Worker).
  • Require subspecialty consultants other than family practice more than twice a year on a chronic basis.
  • Requires modifications of the educational curriculum or the use of behavioral management staff.
• Requires adaptive equipment.
• Requires Assistive technology devices or services.
• Requires wheelchair accessible/housing modifications.
• Has or requires an Individualized Education Program (IEP).
• Has or requires an Individualized Family Service Plan (IFSP).

Ansonsten wird man doch eigentlich hoechstens daraufhin gescreened um auszuschliessen, dass man darunter faellt.

also garcia mein schatz und ich haben aj acuh erst vor 2 wochen geheiratet.

ich hatte allerdings nur ein EFMP screening, bei dem sie ausschließen das du gewisse krankheiten hast, das habe ich benötigt für command sponsorship.
das einzigste was ich dafür benötigt habe war ein ärztliches attest von meinem hausarzt in dem steht das ich frei von ansteckenden krankheiten bin usw.
dazu bekommt man zwei fromulare, das eine füllst du aus und unterschreibst, das andere füllt dein mann aus und dieses muss von deers unterschrieben werden.
danach gibst du die formulare4 ab, eventuell fragen sie dich ein paar sachen (bei mir war das nicht der fall) und dann habe ich am nächsten tag die formulare unterschrieben bei denen abgeholt. das ganze hat genau 10 minuten insgesamt gedauert.

hoffe konnte helfen auch wenn ich keinen termin so wie du hattest. vllt musst du das aber auch aus anderen, oben gennanten, gründen machen!!

mein mann is im iraq ;=( aber ich habe ne power of atterny ;=)
ich hab aber hier in nuernberg keinen arzt... bin nicht so ein ärztefreund...
nun ich hab am 4.8. den termin in der health clinic und habe bereits die form 5888 und die andere ich glaub 7246 oder so ähnlich.... die eine form davon habe ich bei deers schon unterschreiben lassen... aber ohne ein ärtzliches attest kann ich doch auch hingehen oder

gut also dann hast du ja schon mal die formulare.
die same von der health clinic meinte, das sie ein äztliches attest von einem arzt bräuchten.
vllt rufst du da nochmal an und fragst nach nit das du dahin fährst und wirst gerade wieder weggeschickt nur weil das fehlt.
wenn nicht probiere es bei dem letzten arzt bei dem du warst (allgemein mediziner).
